If you live in another school district and want to OPTION IN your child to Sandy Creek in 2025-2026, the deadline for submitting this form to the Sandy Creek Superintendent is March 15. Option paperwork is required once for EACH Option In student Kindergarten through Grade 12.

Confirm which is your RESIDENT school district, on the GIS website: https://www.nebraskamap.gov/pages/county-assessor-gis.
